At IUPUI, how do I request the repair, activation, or installation of a data jack on campus?

As a faculty or staff member, you can request repair, activation, or installation of data jacks at IUPUI by completing and submitting the Data Jack Request Form (authentication required). Please allow five to seven business days from the time the order is placed to the actual activation of the data jack.

If you have questions about completing the form, have difficulty accessing it, or are a student living in campus housing, contact the UITS Support Center in Indianapolis by phone at 317-274-4357 or send email to  ithelp@iu.edu .
