How can I request a passphrase reset by mail or fax?

If you have forgotten your Indiana University Network ID passphrase and you are unable to reset it using the self-service reset system or by visiting a Support Center walk-in location, you can request a reset via mail or fax.

Print and complete one of these forms:

If a printer is unavailable, create your own form by including all of the following information:

  • The fact that you are requesting a passphrase reset
  • Your IU Network ID username
  • Your 10-digit University ID number (UID)
  • Your full name (last, first, and middle initial)
  • Your mailing address
  • Your telephone number (including area code)
  • Your signature
  • The current date

Send your request to:

Alternatively, you may fax your request to 812-855-0354.

Your request will be processed within two business days of receipt in the Accounts Administration office. Your new passphrase will be mailed to the address you include in your request.
