Activate a telephone line in campus housing at IU Bloomington or IU Indianapolis

To activate a student line, a campus housing representative must submit the Analog Line Request form:

1. Under "Contact info", enter the student's contact information:
   • Contact First Name: Enter the student's first name.
   • Contact Last Name: Enter the student's last name.
   • Contact Username: Enter the student's IU username.
   • Contact Phone (10-digit): Enter the student's contact phone number.
2. Under "Request info", enter information about the request:
   • Campus: Use the drop-down to select the appropriate IU campus.
   • Account Number: Enter None.
   • Request Detail: Provide the following:
     • Name of the student's residence hall
     • Student's location (which wing, floor, room?)
     • Jack ID (the telephone jack in the student's room should have a label with the jack ID on it)
3. Click Submit.
