About FireForm at IU

On this page:

Overview

FireForm is the UITS-recommended, self-service form-building tool provided to any IU unit or department without fee. FireForm allows you to build your own forms for data collection and workflows to manage form review and processing. It incorporates two-way integration for moving data between systems and includes payment gateways for simple payment scenarios.

FireForm was created by UITS staff, based on the needs of IU constituents. It is supported by the Enterprise Business Process Solutions (EBPS) and Enterprise Application Systems (EAS) teams. Development will be ongoing so faculty, staff, and students can contribute to enhancements that facilitate their business processes and further the goals of the university.

FireForm is approved for use with PHI and up to Critical data. Throughout FireForm development, the ability for IU constituents to create forms and workflows that are accessible to all users has been a priority per IU's Non-Discrimination/Equal Opportunity/Affirmative Action (UA-01) policy and Americans with Disabilities Act Policy (UA-02).

Note:
The EBPS and EAS teams provide 24-hour support for FireForm performance issues. General inquiries will be answered during normal business hours (Monday-Friday 8am-5pm). The FireForm maintenance window occurs daily from 4-6am.
