How can I request a passphrase reset by mail or fax?

If you have forgotten your Indiana University Network ID passphrase, and you are unable to reset it using the self-service reset system or by visiting a Support Center walk-in location, you can request a reset via mail or fax.

Print and complete the Accounts Administration Passphrase Change Form (in PDF format).

If you're unable to print the form, use a clean sheet of white paper to create your own; include all of the following information:

  • The current date
  • Your full name (last, first, and middle initial)
  • Your 10-digit University ID number
  • Your request for a passphrase reset
  • Your IU username
  • Your telephone number (including area code)
  • Your alternative (non-IU) email address
  • Your mailing address
  • Your signature

Mail or fax your passphrase reset request to:

Fax: 812-855-0354 (attention VALID)

Your request will be processed within two business days of its receipt in the Accounts Administration office. Your new passphrase will be mailed to the address you include in your request.
